Detailed Overview

The Clinical Pharmacist provides clinical pharmacy services including individualization of patient medication therapy, patient medication counselling, consultation with physicians, nurses and other healthcare providers to develop and discuss therapeutic plans, and assistance in the development of drug therapy guidelines for individual disease conditions. They support the Pharmacy & Therapeutics Committee, contribute case studies and articles for professional journals, act as a department resource person, participate in medication use evaluations and clinical drug trials, provide instruction and/or supervision to pharmacy students and residents, and perform drug distribution duties including prescription validation and dispensing.

Responsibilities

  1. Consult with physicians, nurses and other healthcare providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clinical responses; ensures appropriate documentation is maintained in the patient health record.
  2. Individualize patient medication therapy by reviewing literature, patient charts, laboratory results, and patient/health‑care team assessments; modify or develop therapeutic plans.
  3. Provide patient medication counselling regarding drug therapy and demonstrate procedures and equipment.
  4. Assist the health‑care team in developing drug therapy guidelines for individual disease conditions, reviewing current literature, and monitoring guideline effectiveness.
  5. Participate in ward rounds, program team meetings and provision of pharmacy services at the unit level to enhance patient care.
  6. Serve as department resource person in the assigned area of responsibility and provide detailed drug information on issues related to drug therapy in those disease states.
  7. Contribute case studies and review articles to professional journals by identifying topics, gathering data, reviewing literature, writing and submitting articles.
  8. Provide instruction and/or supervision to pharmacy students and residents, preparing educational material, demonstrating procedures, lecturing, supervising patient care activities, and evaluating performance.
  9. Participate in quality improvement activities, performing quality assurance audits and recommending changes.
  10. Perform drug distribution duties, including interpreting medication orders, dispensing medications using a computerized system, and reviewing prescriptions for therapeutic suitability.
  11. Communicate with physicians, patients and other health‑care team members for prescription clarification and problem resolution; document outcomes in the patient record.
  12. Ensure that medications are efficiently and accurately dispensed or compounded and labeled; maintain dispensing records.
  13. Participate in the collection of data for medication use evaluation projects and document required information.
  14. Assist in maintaining adequate inventories, noting shortages, requisitioning drugs and supplies, monitoring expiry dates, and reporting procedural errors.
  15. Participate in clinical drug trials by dispensing medications and completing associated documentation.
  16. Participate in implementation of medication safety initiatives and quality control activities.
  17. Participate in staff meetings, committees and other programmes; identify opportunities to improve processes and refer to supervisor or others.
  18. Maintain records for narcotic and controlled drugs, special access program drugs and departmental statistics in accordance with legal standards.
  19. Prepare reports on a variety of activities within the discipline, gathering information into approved formats and updating spreadsheets.
  20. Perform other related duties as assigned.

Qualifications

Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.

Skills and Abilities

  • Broad knowledge of organization policies, procedures and standards of care.
  • Knowledge of evidence‑based clinical pharmacy practice.
  • Knowledge of medication use evaluation.
  • Knowledge of research processes and methodology.
  • Knowledge of drug distribution processes in a hospital setting.
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain effective working relationships with other health practitioners, patients, family members and the public.
  • Ability to organize and prioritize work.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
