Salary
The salary range for this position is CAD $51.37 - $64.19 per hour.

Responsibilities
- Provide individualized pharmacotherapy and counsel patients about their medication therapy; work in collaboration with physicians and other health care providers to develop, implement and monitor therapeutic plans.
- Assist in the development of drug therapy guidelines for individual disease conditions.
- Prepare and submit case studies and articles for publication to share new knowledge.
- Act as department resource person within your assigned area.
- Participate in medication use evaluations, quality improvement initiatives and clinical drug trials.
- Provide mentorship and supervision to pharmacy students, residents and new staff.
- Ensure prescriptions are validated and dispensed accurately; address medication safety concerns, work with the health care team to resolve discrepancies; document professional practice activities proficiently.

Detailed Overview
The Clinical Pharmacist provides clinical pharmacy services including individualized patient medication therapy; patient medication counseling; consultation with physicians, nurses and other health care providers to develop and discuss therapeutic plans, and assists in the development of drug therapy guidelines for individual disease conditions. Provides support to the Pharmacy & Therapeutics Committee by preparing summaries and recommendations; contributes items such as case studies and articles for publication; acts as department resource person; participates in medication use evaluations and clinical drug trials; provides instruction and supervision to pharmacy students and residents. He/she also performs drug distribution duties including processing prescriptions, validating prescriptions, identifying safety concerns, and resolving discrepancies; documents discussions or modifications to therapy and dispensing medications.
Qualifications
Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.

- Broad knowledge of organization policies, procedures and standards of care.
- Knowledge of evidence-based clinical pharmacy practice.
- Knowledge of medication use evaluation.
- Knowledge of research processes and methodology.
- Knowledge of drug distribution processes in a hospital setting.
- Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision.
- Ability to communicate effectively both verbally and in writing.
