Overview

Clinical Resource Nurse, Community Programs - Tri-Cities – Fraser Health Authority

Salary: CAD $49.20 - $67.08 / hour

Location: Port Moody, British Columbia, Canada (Home Health)

Responsibilities

  • Provides clinical expertise, leadership, education and consultation to staff and/or clients in the designated clinical area; develops, implements, revises and evaluates clinical treatment modalities and standards; coordinates educational plans with staff to maximize resources; participates in quality improvement, risk management, research and special projects; collaborates with interdisciplinary teams to ensure clinical objectives are met.
  • Role models direct client care skills to staff; ensures compliance with standards of practice and supports staff to provide quality client care based on specialized theory and best practice.
  • Leads and provides consultation to staff on implementing and evaluating client care plans; facilitates collaboration with other health care professionals and community resources; provides staff with referral options.
  • Promotes client self-care, illness prevention, health maintenance and health promotion strategies; evaluates outcomes and maintains current reference materials.
  • Develops and revises delegation of task procedures; provides leadership in implementing and evaluating delegation of tasks as required.
  • Develops, implements, evaluates and revises education, orientation and professional development programs; conducts learning needs assessments and introduces new skills based on current theory, research and standards of care.
  • Identifies resources to assist in managing complex client/family health issues and makes recommendations to leadership as required.
  • Develops and/or modifies course curricula for teaching groups to meet educational needs and evidence-based practices.
  • Participates in quality improvement and risk management activities; supports changes in practice with consultation from leadership as required.
  • Participates in development and dissemination of information, standards, policies, procedures and guidelines for the designated clinical area.
  • Participates in research and special projects; collects and interprets data and provides input for further analysis as required.
  • Sets measurable professional goals and participates on professional committees to advocate for the designated clinical area.
  • Contributes to staff recruitment and performance evaluations as requested.
  • Performs other related duties as assigned.

Qualifications

Education and Experience
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years\' recent related clinical experience in the designated clinical area, including one (1) year experience in an educational/leadership role, or an equivalent combination of education, training and experience. Completion of post-graduate course work and/or certification applicable to the designated clinical area.
Valid BC Driver\'s license and access to a personal vehicle for business-related purposes.

Skills and Abilities
Demonstrated knowledge of Home Support policies, adult learning principles, nursing theory and evidence-based practice; ability to develop, implement and revise education/training programs; leadership through coaching and mentoring; effective communication; ability to work independently and in interdisciplinary teams; ability to plan, organize and prioritize work in a dynamic environment; physical ability to perform duties.
