Salary Range : $64,222 - $80,277

Summary

To lead intake and employment services while serving as the primary point of contact for individuals and families, coordinating intake, case management, advocacy, and transition planning in collaboration with community partners and internal teams.

Key Activities

Collaborate with Community Partners, Stakeholders & Family Members

  • Work effectively with community partners to identify individuals in need of DS and ABI service and conduct comprehensive intake interviews to support service planning.
  • Work with DSO, Ontario Health atHome, Service Navigators and Community Partners to identify individuals requiring services.
  • Serve as the front-facing first point of contact for individuals, families, and service providers, offering responsive guidance, information, and support to facilitate access to appropriate services.

Coordinate Assessments & Interviews

  • Pre-assess individual support needs match available resources and schedule intake assessments.
  • Conduct in-depth intake interview and collect personal intake information, including but not limited to authorization and consent to release information, financial information, medical history.
  • Maintain Client Records and Documentation

  • Maintain accurate client records and required documentation while collaborating with managers, clinicians, and frontline staff to develop effective transition plans.
  • Complete all required internal documentation (intake reports, financial documentation, etc.) to ensure a successful transition to care.
  • Work with Client Service Managers, Clinical Consultants and frontline staff in the development and execution of transition plans to further ensure successful transition to service.
  • Administration and Maintenance of MCCSS-Funded Resource Databases

  • Oversee the administration and maintenance of MCCSS-funded resource databases.
  • Collaborate on individualized budgeting, and manages all reporting of service changes, funded resources and vacancies.
  • Understand, interpret and maintain MCCSS databases for all funded resources at Pathways to Independence.
  • Work with clinical director to create individualized budgets and implementation plans when creating capacity for new supported individuals.
  • Case Management

  • Act as the primary point of contact and provide case management, advocacy, and coordination of community and OPR supports for individuals and families.
  • Provide oversight and Case Management for Pathways supported individuals currently residing in Outside Private Resources (OPRs).
  • Provide Case Management, as required, for Pathways supported individual’s residing in the community and act as a liaison and advocate for them and their families for the purpose of responding to concerns and sharing information.
  • Employment Program

  • Lead and oversee employment programs that support adults with developmental disabilities to obtain and maintain meaningful, community-based employment.
  • Supervise, mentor, and support Employment Facilitators by providing guidance on best practices in supported employment, customized employment, and community inclusion.
  • Facilitate frontline staff to build and sustain positive relationships with inclusive employers, community agencies, families, and service system partners to expand employment opportunities and support long-term job success for individuals receiving services.
  • Ensure compliance with developmental services legislation, funding agreements, and policies; monitor outcomes, maintain accurate documentation and reporting, and use data and feedback to strengthen program quality and effectiveness.
  • The successful candidate must have the following :

    Qualifications :

  • Social Services Worker Diploma or Human Services-related Degree from an accredited college or university.
  • Valid driver’s license and reliable transportation required.
  • Bilingual in English and French preferred.
  • Role Related Skills :

  • Excellent communication skills, both written and oral, and ability to adjust communications to fit the needs and level of understanding of the receiver.
  • Compassion and understanding of varied individuals from diverse backgrounds with a consistent, patient, diplomatic and responsive approach to a variety of circumstances
  • Ability to exhibit exemplary interpersonal skills, including active listening and clarifying, to develop rapport with individuals, families and support networks.
  • Excellent ability to solve problems by conducting analysis of information, evaluation options, formulating conclusion and implementing solutions in keeping with the organization’s mission.
  • Exercise sound judgment, maintain confidentiality and professionalism.
  • Excellent attention to detail.
  • Highly proficient computer skills (Microsoft Suite, Database).
  • Experience : 3-5 years in a related field, with considerable knowledge of the community and human services sector.
