Responsibilities - YOUR Role

As a LPN Clinical Lead at Pro Vita Care Management , your role will be pivotal in ensuring the health and happiness of our residents.

Your day-to-day activities will include :

  • Providing strong leadership to the care department.
  • Ensuring that our services align with the Mission, Vision, Values, and Philosophy of Pro Vita.
  • Developing and delivering training programs and formulating individual action plans.
  • Monitoring nursing practices to uphold the highest standards of care.
  • Maintaining important relationships with senior-level contacts internally and externally to enhance our care programs.
  • Collaborating closely with physicians, pharmacists, and other healthcare professionals to ensure seamless delivery of integrated care for our residents.
  • Performing Medication Reconciliation and Admission / Discharge Audits.
  • Participating in discharge planning activities.
  • Planning and arranging transports for up-coming appointments on the resident’s behalf where required.

  • Graduation from a recognized Practical Nursing program
  • Active registration with the BCCNM as an LPN
  • Occupational First Aide Level 1 certification
  • 2 years of Gerontological or Assisted Living experience
  • Satisfactory proof of COVID-19 vaccinations
  • RAI MDS Education (Gold Care or Point Click Care)
  • Effective communication skills in English
