Job description

About the Opportunity

The Care Coordinator is responsible for assessment, care planning, authorization of services, and ongoing case management of clients referred to Continuing Care. The coordinator helps identify goals and formulates plans to meet identified client needs.

They authorize services, make referrals and linkages to other professional and volunteer resources, and work with interprofessional teams to facilitate hospital discharge to return to the community.

Care Coordinators work in community, hospital, or long‑term care facilities. They are fiscally responsible for the care, services, and equipment authorized within available policies and programs, maintain ongoing communication with clients, family, and health care teams, and manage reassessments at routine intervals.

About You

We are looking for candidates who meet the following qualifications :

  • Bachelor’s Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies
  • Current registration (or eligibility) with the applicable Nova Scotia Professional Regulatory body
  • Minimum 3 years of related experience
  • Demonstrated knowledge / experience in a community or long‑term care setting
  • Experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and cooperatively in a busy multidisciplinary setting
  • Ability to multitask, prioritize workload, and manage time efficiently
  • Valid driver’s licence and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset

Hours

  • Full‑time, long assignment; 75 hours bi‑weekly
  • Approximate one‑year assignment
  • Shifts may include days, evenings, weekends, and holidays
  • Compensation and Benefits

    $46.46 – $54.22 hourly

    Successful candidates may be eligible for a benefits package that includes health, dental, travel, long‑term disability and life insurance coverage, as well as a defined benefit pension plan.
