About the Opportunity
The Care Coordinator is responsible for assessment, care planning, authorization of services, and ongoing case management of clients referred to Continuing Care. The coordinator helps identify goals and formulates plans to meet identified client needs.
They authorize services, make referrals and linkages to other professional and volunteer resources, and work with interprofessional teams to facilitate hospital discharge to return to the community.
Care Coordinators work in community, hospital, or long‑term care facilities. They are fiscally responsible for the care, services, and equipment authorized within available policies and programs, maintain ongoing communication with clients, family, and health care teams, and manage reassessments at routine intervals.

Compensation and Benefits
$46.46 – $54.22 hourly
Successful candidates may be eligible for a benefits package that includes health, dental, travel, long‑term disability and life insurance coverage, as well as a defined benefit pension plan.
