JOB SUMMARY
The primary areas of responsibility include health promotion, health education, and community health development. The CHN uses independence and initiative to plan, organize, implement, and evaluate community health services, providing care in accordance with established nursing standards, departmental objectives, and all applicable Acts and Regulations. The CHN ensures that Public Health Nursing services are efficiently coordinated and accessible and actively participates as a team member in delivering quality programs and services.
REPORTING STRUCTURE
Reports to the Director of Health. Clinical oversight is provided by the Integrated Care Manager.
JOB DUTIES AND RESPONSIBILITIES
Identifies needs and provides health services in homes, clinics, schools, and offices to individuals and groupsAssesses care needs and educates individuals, groups, and the community through health programming sessions and events (e.g., diabetes, pre‑ and post‑natal care, tuberculosis and other screenings)Provides health education and promotion services to support healthy lifestyle changesProvides education regarding protection against communicable diseases, including administering immunizationsPlans, organizes, implements and evaluates community health services to promote wellness and prevent illness and diseaseAnalyzes community health indicators, such as lab reports, communicable disease reports and statisticsProvides treatments, support and follow‑up to ensure complianceDevelops action plans for individuals, families, schools and the community for health emergencies and pandemic responseSelects and implements services and activities that support positive health outcomesAdvocates for and refers community members to appropriate agencies, medical providers and programsSupports coordination of client visits with physicians, hospitals, physiotherapy, occupational therapy, dental, optometry and audiology servicesLiaises with physicians, hospitals, teachers, social workers and other community service agenciesIdentifies health issues and needs by analyzing demographic, health and related dataProvides services and establishes methods to measure effectivenessMaintains statistics (treatments, immunizations, etc.) to support health planningCoordinates Public Health Nursing services with community agenciesArranges and participates in joint home visits with other professionalsAssists with planning and development of community health resources, including emergency and pandemic planningMay be assigned emergency services duties during community emergenciesParticipates in community events as requestedAll persons employed by the Okanagan Indian Band may be required to assist the Band in providing emergency services. Develops community health work plansCreates health promotional materials and educational displays as part of the teamCompletes community health reporting, including PanoramaConducts home and office visitsAttends professional development to maintain current knowledge and skillsAttends community meetings and workshops relevant to the CHN rolePromotes traditional and cultural health practicesPrepares course materials and delivers sessions (e.g., expectant parents, parenting, weight management, elder health)Ensures all care complies with relevant policies, procedures and regulationsMaintains a current, accurate, and confidential client reporting and filing systemMaintains vaccine integrityOversees biohazard managementParticipates in pandemic, emergency and community planningReports concerns to the Director of HealthPerforms all duties in accordance with OKIB policies and within BCCNM scope of practicePerforms other duties as requiredWHO ARE WE LOOKING FOR
You have education, experience, or an equivalent combination may be considered. You are community-minded, detail-oriented, have a willingness to contribute to a team and an excellent communicator.
Education
Bachelors degree in nursing (RN) from a recognized universityCurrent, active registration with BC College of Nursing and Midwives (BCCNM)Current First Aid / CPR certificate with AED or willing to obtainValid BC Driver’s license, minimum Class 5 and reliable vehicleCriminal Record Check with Vulnerable Sector SearchImmunization Certification and knowledge / training and / or experience in Managing Communicable Disease within a community settingExperience
Minimum 2 years of experience working in a community settingExperience working with First Nations communitiesStrong team / client leadership, communication, problem solving and teamwork skillsExperience with program planning, delivery and facilitation an assetCompetencies (KSAs & Desired Attributes)
