Job Description

Summary

Reporting to the Patient Care Manager, the Staff Nurse, assesses, identifies, plans, implements and evaluates the nursing care required to assist patients / residents in meeting their physical, social, spiritual and psychological needs using an established framework for nursing practice.

Develops, implements, evaluates and revises the plan of care. Incorporates individual and / or group teaching into the care plan and nursing interventions and / or makes referrals to other identified resources.

Supports other nurses and health care professionals by collegially sharing information and demonstrating procedures. The Staff Nurse works within an interdisciplinary clinical framework in accordance with the BCCNM Standards of Practice and consistent with the mission, vision and values and established protocols of Providence Health Care.

Qualifications / Skills and Education

Education, Training and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives and currently practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.

Advanced preparation in GI Endoscopy that includes completion of recognized post-basic education in gastroenterology / GI endoscopy nursing and a certificate of completion from the Association of Perioperative Registered Nurses (AORN) in gastroenterology, or an equivalent combination of education, training and experience.

Preference given to those with recent related GI endoscopy experience.

Duties and Responsibilities

1. Reviews patient status through methods such as reviewing the patient chart and other written information, observing patient, interviewing patient and family members, completing questionnaires, gathering collateral information and consulting with peers and other members of the health care team.

2. Completes a comprehensive assessment of the physiological, psychological, socio-cultural and spiritual needs of the client and family unit and determines client status and responses to actual or potential health problems.

Develops the plan of care for patients using clinical judgement and evidence-based practice ensuring the client and family actively participates in the development, implementation and ongoing modification of the plan of care, including the long and short-term goals.

Develops and implements learning plans to meet identified client learning needs.

3. Coordinates and oversees the overall care. Sets priorities, provides clinical expertise and leadership for the plan of care and implements health care treatments and interventions.

Assigns and delegates tasks as appropriate to other members of the healthcare team. Evaluates the appropriateness and effectiveness of treatments and interventions on an ongoing basis.

4. Establishes a therapeutic relationship using effective interpersonal and interviewing techniques, ensuring client choice and autonomy in decision-making and care planning.

Acts as a role model by demonstrating empathy, trust, and respect in all interpersonal relationships and recognizes personal impact on team dynamics.

5. Monitors patient progress, evaluates outcomes, reassesses if goals are not met and modifies plan of care based on evaluation.

6. Documents assessments, records of observation, care provision and client responses according to professional standards and PHC guidelines, which may include computerized records and databases.

Reports problems and / or changes in patient condition and ensures pertinent information and significant clinical changes are communicated to the client, family and members of the multidisciplinary team.

7. Provides direct nursing interventions for management of crisis and life threatening situations.

8. Accompanies patients in medical transport vehicles as required by the employer.

9. Identifies and responds to patient and family learning needs, provides patient educational and reference materials, incorporates individual and / or group teaching activities into the care plan and nursing interventions and / or makes referrals to other identified resources.

Leads group patient / family sessions as required and coaches patient / family to develop and maintain health promoting behaviours within the context of the care issue.

10. Provides direction to other staff such as Care Attendants and Licensed Practical Nurses with regard to patient care and facilitates effective teamwork.

Responsibilities include but are not limited to collegial sharing of information, demonstrating and / or providing guidance on related tasks, providing incidental or ad hoc input into staff performance, ensuring staff are aware of assignments and reassigning clinical / health related tasks.

11. Facilitates discharge plans and liaises with families and community resources agencies.

12. Participates in nursing orientation and shares clinical knowledge with other nursing staff, acts as a preceptor or mentor to guide the clinical experience of assigned new nursing staff and students and other members of the interdisciplinary team.

Provides input to the Nurse Manager as requested on individual’s progress in achieving established standards of care.

13. Participates in planning, implementation and evaluation of patient care policies, procedures, education materials, and best practices through methods such as taking part in program and quality improvement activities including nursing practice council, clinical pathway development, research and education and other committees.

14. Maintains a safe environment for clients, family, and staff, through :

a. Accident prevention,

b. Knowledge of emergency procedures and regulations,

c. Using skills, physical facilities, supplies and equipment safely and appropriately, and

d. Practicing infection control procedures.

15. Operates and / or cleans equipment related to patient care and reports malfunctions to supervisor. Uses appropriate supplies for care in accordance with departmental procedures.

Handles, stores and disposes of workplace hazardous materials ad sharps in accordance with established procedures.

16. Participates in and attends in-service presentations and other educational programs as required. Supports nursing research through methods such as identifying areas for possible clinical nursing research projects, adapting research to practice and participating in research activities to enhance nursing practice.

17. Advocates for the patients to protect and promote their rights to dignity, respect, privacy, confidentiality, autonomy and access to information.

18. Performs other related duties as assigned.
