Duties
- Work with an adult population with a serious mental illness (schizophrenia and related disorders) and their families
- Assessment, treatment, and support to clients and families
- Complete risk assessment, crisis intervention plans, and clinical needs assessment
- Provide education and treatment in individual and group settings for patients and families
- Liaise with appropriate residential settings and community resources
- Discharge planning, in collaboration with the clinical team and families
- Advocate on behalf of clients and families for appropriate services and resources
- Engage client, family, and significant others in the clinical interventions and risk reduction strategies with the interdisciplinary team
- Develop and leadgroups
- Attend monthly Social Work discipline meetings
- Attend and participate in monthly Social Work Peer Consultation Groups
- Assist with planning and implementation of Community Treatment Orders
- Demonstrate thorough knowledge of the theories and practices of a professional social worker
- Document on patient’s clinical record according to hospital, College, and legislative standards
- Maintain work load measurement and data collection (. RAI) as required
- Participate and lead in research and program evaluation
- Participate in supervision, teaching and education of Social Work students, and support students from health related disciplines
- Participation in hospital Social Work and quality improvement requirements
- Keep current of hospital policies and procedures and applicable legislation
- Keep up to date regarding professional standards by reviewing current literature, attending educational events, networking with colleagues, etc.
- Participate in Program and discipline specific committees and task groups
- Provide educational presentations to community groups
- Work in a manner that is in compliance with staff and patient / client safety practices, policies and procedures of the Royal

Qualifications :
- Masters degree in Social Work
- Registration and in good standing with the Ontario College of Social Workers and Social Service Workers
- Minimum of three (3) years clinical experience in a mental health setting
- Discharge planning experience and knowledge of community resources is essential
- High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
- Experience in group work preferred
- Knowledge of Cognitive and / or Dialectical Behavioural Therapy is preferred
- Client centered approach
- Experience with recovery-based programs for the seriously mentally ill is an asset
- Experience working with Concurrent Disorders population asset
- Conflict resolution knowledge
- Ability to work as a member of an interdisciplinary team
- Valid drivers license and access to a vehicle
- Willingness to work variable hours, evenings and weekends
- English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression.Bilingual (French / English) is considered an asset.
Salary Range : $44.67 to $57.90 per hour
