Bayshore’s Integrated Care Solutions team is seeking a Manager of Clinical Practice to support our coordination teams, provide clinical expertise, and ensure smooth day to day operations across multiple programs. In this office based leadership role, you will act as a key resource for escalations, workflow support, and clinical decision making, helping our teams deliver safe, timely, and compassionate care to clients and caregivers.
What You’ll Do
•Act as the primary in office clinical resource for coordination teams, offering guidance on clinical questions, care pathways, and best practices.
•Support program overflow by assisting with intake reviews, follow ups, and urgent client needs.
•Manage daily escalations from coordinators, clients, caregivers, and partners, ensuring timely and clear resolution.
•Provide cross program backup to maintain continuity during staff absences or high volume periods.
•Review client files, care plans, and documentation for accuracy, completeness, and compliance with Bayshore and provincial standards.
•Assist coordinators in identifying risks, prioritizing cases, and ensuring safe, appropriate service delivery.
•Contribute to quality improvement initiatives by monitoring key metrics and escalating issues as required.
•Maintain strong working relationships with hospital discharge teams and community partners.
•Support follow up on hospital referrals, discharge clarifications, and transition of care inquiries.
•Assist in coordinating safe, timely discharges by ensuring all clinical and logistical requirements are met.
•Provide real time coaching and mentorship to coordination staff to strengthen clinical judgment and decision making
Job Qualification
What You Bring
•5+ years in healthcare, ideally in home care and hospital settings.
•Registered Practical Nurse with current CPR certification.
•Strong knowledge of community nursing, discharge planning, and Ontario’s health system.
•Proven leadership and mentorship skills.
•Familiarity with RAI tools (an asset).
•Valid driver’s license and reliable vehicle.
