What will you do?

The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for Ontario Health atHome patients in a variety of settings (in homes and retirement homes).

Primary Responsibilities

• Provides physiotherapy services to Ontario Health atHome patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient’s condition.
• Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.
• Ensures that clinical records are maintained in accordance with Ontario Health atHome policies and procedures and the College of Physiotherapists of Ontario.
• Schedules patient visits according to Physiotherapy priorities.
• Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals.
• Acts as an education and resource person for other health care professionals.
• Participates in committees as approved.
• Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy.
• Maintains supplies and recommends equipment for department use within budget limitations.
• Interprets the role and objectives of the Ontario Health atHome Physiotherapy Program in the community.

Knowledge of health care system and the role of the Ontario Health atHome Champlain within the system, knowledge of Ontario Health atHome Champlain operations, quality, risk, and performance management principles, knowledge of relevant health care legislation, knowledge of Ontario Health atHome Champlain policies, procedures, strategic direction and goals, understanding of Ontario Health atHome Champlain partners and contracted service providers in the community.

Skills and Abilities

Effective planning and organizing skills to manage multiple patients, accurately complete required documentation, working knowledge of computer software (email, internet) and Microsoft Office applications, strong interpersonal skills, ability to manage difficult conversations, demonstrated ability to work effectively in both a team environment and independently, effective communication skills, demonstrated strong customer service skills, effective listening and facilitation skills, ability to deliver information effectively in various settings, valid driver’s license and access to a vehicle, fluency in English and French is an asset.

Hours of Work

Monday to Friday – 8 : 30am to 4 : 30pm

Starting Salary

$44.480 / hr to $51.340 / hr
