Job Description

Are you a passionate and adaptable nurse looking to make a real impact in the community? We're seeking a dedicated SWAN Nurse to support all of our Community Programs!

Bayshore HealthCare is one of Canada’s leading providers of home and community health care services and is a privately owned company. Bayshore HealthCare is proud to showcase its achievement as a Platinum member of Canada’s Best Managed Companies Program every year since 2006. Bayshore HealthCare is also recognized as one of Canada’s Best Employers in Forbes' 2023 list.

Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.

If your calling is to be the “difference” by providing outstanding care, then a health care career with Bayshore is the perfect fit for YOU!

We value our Bayshore Registered Practical Nurses with:

  • Paid travel
  • Full and part-time careers
  • Highest industry wages for the critical work you do
  • Company-sponsored benefits (health and dental) for you and your family
  • Retirement savings plan with a company contribution
  • Paid sick days, paid orientation, and paid training
  • Competitive travel compensation
  • Efficient work schedules with more care time and less travel
  • Opportunities to grow in your career
  • Company-provided mobile devices for easy documentation and strong communication
  • Easy access to managers and schedulers who listen and support the important work you do!

Independent Contractor Opportunities Available
For qualified professionals who prefer to work as independent contractors, we offer flexible assignments, schedules and competitive per-visit or hourly rates.

Job Qualification

Qualifications you currently have:

  • Completed CNA-accredited Skin Wellness Associate Nurse course (SWAN) or International Interprofessional Wound Care Course (IIWCC)
  • Registered Practical Nurse who graduated from an accredited School of Nursing
  • Registered as a member in good standing with the College of Nurses of Ontario
  • Must have a valid driver’s license and reliable vehicle
  • Two years of previous experience in the development of wound care management programs in a hospital or community setting
  • Previous visiting nursing experience will be considered an asset
  • Updated immunization records, clear criminal background check, and vulnerable sector check upon hire
  • Current CPR certification required

As Canada’s largest and best homecare company, join us as we continue our 50+ year tradition of supporting our Nurses to make “being the difference” a Career!
