Job Summary :

The Nurse Practitioner (NP) is a leader within a multidisciplinary and inter-professional team. Working as a Registered Nurse in the Extended Class RN (EC), the NP will formulate clinical decisions and develop adult client care plans for patients with chronic kidney disease (CKD).

The Nurse Practitioner works in partnership with all relevant stakeholders, excelling in all four domains of their practice : clinical care, leadership, education and research.

The NP will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients in the Nephrology program.

This will involve health assessment and diagnosis, health care management and therapeutic intervention. The NP will demonstrate in practice, the competencies to use their legislated authority to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform specific procedures.

The NP role works in collaboration and consultation with medical staff, nursing and members of the inter-professional health care teams to ensure a timely response to a client’s changing health status and needs.

As an integral team member of the Nephrology Program, the NP will assist the Program by utilizing a Chronic Disease Prevention and Management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan III.

The NP will participate in and lead quality improvement and administrative activities focus on advancing patient care outcomes.

Other key components of the role include performing research activities; developing, delivering, coordinating and participating in education and professional development activities / programs;

performing activities to further develop and strengthen the profession; acting as a resource and mentor; and serving as a consultant to individuals and groups within the professional community and other hospitals / agencies.

The incumbent will be the key liaison with the Nephrologists, primary care, referral partners and the community to ensure seamless transitions, in and out of the Nephrology ambulatory clinics, facilitating collaboration among all partners involved in the ongoing care of the client.

Primary work will be in the In Centre Hemodialysis unit.

Responsibilities :

  • Providing a specialty-based focus, functions in an extended class nursing role demonstrating advanced assessment skills and sound knowledge of clinical therapeutics to formulate clinical decisions and provide comprehensive patient care for clients with CKD;
  • Facilitating interdisciplinary care by participating in regular team meetings, providing a linkage between the client, members of the interdisciplinary team, physicians and other members of the health care team;
  • Liaison with community-based providers, advocating on behalf of clients and families and participation in patient and family conferences as needed;
  • Acting as a role model and resource for expertise in clinical nursing practice in the area of CKD to members of the interdisciplinary team;
  • Providing the key liaison role with the client’s primary care provider, referral partners and the community to ensure seamless transitions in an out of the Nephrology clinics, facilitating collaboration amongst all partners;
  • In collaboration with other members of the health care team, identifying and analyzing the learning needs of Nephrology staff;
  • teaching staff, clients, family and other inter-disciplinary members issues related to CKD and associated comorbid care management modalities;
  • Providing internal and external nursing consultation to clients / families, health professionals, groups and organizations related to CKD management;
  • Participating in decision-making process at the unit and the regional program level on issues impacting on clinical practice;
  • Participating in advance practice nurse meetings, self-management process, and administrative related activities; provides indirect patient care services to support patient care, promote professional education, and advance knowledge through research and scholarly activities;
  • Attending clinics, patient care rounds and educational sessions for patients with CKD conditions as required;
  • Participating in the design, implementation and evaluation of continuous quality initiatives;
  • Collaboration with other NP’s at other Nephrology Programs / sites within Ontario.
  • The NP will formulate clinical decisions and develop adult client care plans for patients with glomerular disease. The incumbent will be the key liaison with the Nephrologists, primary care, referral partners and the community to ensure seamless transitions, in and out of the Nephrology ambulatory clinics, facilitating collaboration among all partners involved in the ongoing care of the client.

Primary work will be in the GN Clinic; however, work across the program and in all service areas will be required depending on the emerging priorities

Requirements :

  • Current registration in the Extended Class with the College of Nurses of Ontario required
  • Masters Degree in Nursing required
  • Nurse Practitioner Certificate required
