Reports to :   Nurse Manager

Work Model : OnSite

Grade :  F0 : 03

Wage Range : $44.51  55.64 per hour (to commensurate with experience and consistent with UHN Compensation Policy)

Hours : 37.5 Hours per Week

  • Shifts :   8 Hour Days;

Status : Permanent FullTime

Job Summary

The Princess Margaret Cancer Centre has 12 site groups and 26 specialty clinics and almost 3000 staff who see over 400000 patient visits every year. Its 800000 square feet house 130 inpatient beds 373000 square feet of research space and 17 radiation treatment machines makes it one of the largest comprehensive cancer treatment facilities in the world and the largest radiation treatment centre in Canada.

The Messner Allogeneic Stem Cell Transplant Program is the largest in Canada and one of the largest in North America. Our program delivers approximately 200 allogenic stem cell transplants per year on our inpatient units.

Duties

As an integral member of the interprofessional team the sitebased  Patient Care Coordinator (PCC)  acts as a resource and coleader for the interprofessional disease site team(s). The  PCC  has clinical and leadership expertise. The principal responsibilities within the site(s) include :

  • Ensuring efficient day to day operations of clinical areas including patient flow and patient activities
  • Collaborates with the interprofessional team to enhance the provision of care that incorporates clinical standards best practice and enhances care / service delivery outcomes for patients
  • Contributing to staff professional development and ensuring a supportive / collaborative learning environment within the department
  • Performing human resources responsibilities in collaboration with the Nurse Manager
  • Liaising with members of the multidisciplinary team to ensure high standards of quality and optimal management of patient care outcomes (including data collection / reporting)
  • Qualifications :

  • At minimum a completion of BScN Baccalaureate in Nursing or Baccalaureate level education with Diploma in Nursing required
  • Completion of a Masters degree in Nursing preferred
  • Basic Cardiac life Support (B.C.L.S.) program
  • Minimum 3 years inpatient nursing experience in hematology / oncology
  • Experience in stem cell transplant an asset
  • Current certificate of competency (licensure) from the College of Nurses of Ontario required
  • Demonstrates membership within a professional nursing association and / or nursing interest groups related to practice
  • Demonstrated strong leadership and motivational skills
  • Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
  • Demonstrated leadership abilities
  • Excellent skills in recognizing analyzing and resolving problems
  • Excellent time management skills
  • Excellent decision making and problemsolving skills
  • Clinical and patient teaching skills
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
  • Exercise initiative and good judgement with the ability to multitask

  • Maintains and protects patient confidentiality in accordance with UHN Confidentiality policy requirements
  • Effective organizational interpersonal and communication skills
  • Knowledge of applicable legislative UHN and / or departmental policies
  • Client service oriented with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand
