Located in Hamilton’s downtown core, Hamilton General Hospital provides leading edge care and is home to a number of regional referral programs. The neurosciences and trauma program is a provincial leader in both volume and complexity of care provided. The HGH stroke program has pioneered an integrated approach to care, and our burn treatment program is one of only two in Ontario. The cardiac and vascular program performs more heart surgeries and procedures than any other facility in the province.
The Short Stay Medical Unit (SSMU) is an active adult general internal medicine unit that is a part of our Acute Medicine Program at the Hamilton General Hospital. The Short Stay Medicine Unit (SSMU) works with an interdisciplinary team approach to provide care to an acutely ill, primarily geriatric 65+, patient population, including patients with multi-system chronic illnesses.
Position Summary
Using a client-centered approach, the Registered Nurse (RN) provides leadership in the coordination and evaluation of nursing care of patients with multisystem medical conditions awaiting transition to the community setting. The RN works collaboratively with interdisciplinary team members and community agencies to coordinate care and education of patients and their families. In this fast-paced environment, admitted patients are typically discharged within 72 hours or less. Through evidence-based clinical practice and strong leadership in healthcare delivery, the RN sets the standards of excellence in the care of patients at HHS. The RN establishes and implements education plans with and for patients and families and acts as an accessible and knowledgeable resource for patients and their families.

Rotating days, evenings, nights, weekends, and holidays
Qualifications
Current Registration with the College of Nurses of Ontario as a Registered Nurse
Current Basic Cardiac Life Support (BCLS) certification
HHS’ Critical Practice Orientation (CPO) Telemetry education and experience would be an asset
Minimum one (1) year nursing experience in a medical setting within the last 2 years preferred
Knowledge of the current nursing assessment and management of adult and geriatric medicine patients, including rehabilitative approaches to patient care
Understanding of and commitment to applying principles of Senior Friendly Care
Demonstrated leadership, critical thinking, decision-making, communication, organizational, problem identification and problem-solving skills
Demonstrated ability to work independently while collaborating with an interdisciplinary healthcare team
Demonstrated ability to manage multiple and shifting demands
Commitment to providing Excellent Patient Experiences
Commitment to ongoing learning
Evidence of consistent attendance and punctuality
