Position Overview :
Shkagamik-Kwe Health Centre is seeking a dedicated and compassionate Social Worker to join our team on a full-time, permanent basis. The successful candidate will play a pivotal role in delivering client-centred, culturally safe, and holistic social services. This position is designed for a professional who is committed to fostering community well-being, particularly within diverse populations, including Indigenous communities.
Key Responsibilities :
Conduct comprehensive assessments of clients’ social, emotional, and physical needs.
Develop and implement individualized care plans in collaboration with clients, families, and multidisciplinary team members.
Provide ongoing case management, crisis intervention, and follow-up services.
Offer therapeutic interventions including counselling and support groups tailored to clients facing mental health challenges, substance abuse issues, family conflicts, or other social concerns.
Facilitate empowerment and resilience-building strategies for clients.
Liaise with community resources, healthcare providers, and government agencies to secure additional support and services for clients.
Advocate for client rights and access to social services and community programs.
Provide culturally sensitive care and maintain an understanding of Indigenous cultural practices, values, and protocols.
Collaborate with team members, including healthcare professionals and community leaders, to integrate culturally relevant support into care plans.
Maintain accurate and confidential client records in accordance with privacy legislation and organizational policies.
Participate in regular team meetings, case conferences, and ongoing professional development.
Contribute to the design and evaluation of programs and services that meet the evolving needs of our community.
Provide feedback to improve service delivery and client outcomes.
Qualifications
Bachelor / Master of Social Work (BSW / MSW) from an accredited institution is required.
Registration with the Ontario Association of Social Workers (OASW) or equivalent professional designation.
A minimum of 2 years’ experience in social work practice, preferably in a community health or public service setting.
Experience working with diverse populations, particularly Indigenous communities, is an asset.
Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Skills & Competencies :
Excellent interpersonal, communication, and crisis intervention skills.
Strong understanding of case management, community resources, and advocacy.
Ability to work both independently and collaboratively within a multidisciplinary team.
High level of cultural competency and sensitivity, with a commitment to practicing in a culturally safe environment.
Proficiency in record-keeping and documentation in compliance with confidentiality standards.
Additional Requirements :
Flexibility to participate in outreach activities and community events.
Ability to manage a varied caseload and work under deadlines.
Working Conditions :
This position is based at the Shkagamik-Kwe Health Centre in Sudbury, Ontario.
The role may require occasional travel for community outreach and professional development.
Standard working hours with potential on-call responsibilities as needed to support client care.
