Location : On-Site @ Lifemark Physiotherapy Nature Park Way (76 Nature Park Way, Winnipeg, MB, R3P 0X8)
Status : Casual, part-time or Independent Contractor model @ 4 to 12 hours / month
About PCVRS : Partners in Canadian Veterans Rehabilitation Services (PCVRS) provides comprehensive rehabilitation assessment and intervention for Canadian Veterans experiencing barriers to transition due to service-related injuries or illness. Using a biopsychosocial approach, services are performed by an interdisciplinary team of clinicians managing complex conditions such as chronic pain, head injury symptoms, MSK injuries, often with mental health comorbidities.
The Physician will :
- Complete thorough assessments of eligible health problems;
- Review medications and identify drug therapy issues;
- Clarify medical issues, including comorbidities;
- Determine diagnosis and prognosis;
- Identify unrelated conditions that may hinder rehabilitation;
- Assess medical restrictions and accommodations;
- Recommend additional assessments when necessary;
- Collaborate on treatment plans emphasizing education, coping, and self-management;
- Engage with Veterans, support persons, and families to promote understanding;
- Communicate with primary care providers for coordinated care;
- Provide care in-person or virtually;
- Participate in team debriefs and follow-up care;
- Engage in continuous quality improvement and professional development.
