As an Occupational Therapist (Permanent, Full-Time; Effective : Immediately; Location : Sudbury) , you are responsible for theprovision of occupational therapy services to Ontario Health atHome patients in order to maximize the patient’s function, independence, and safety in their respective environment.
Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.
As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness andwork-life balance.
What will you do?
- Assesses the factors influencing the patient’s ability to function in their environment, using a variety of standardized and non-standardized assessment tools.
- Determine patient centered therapy goals and provides treatment program to increase occupational performance, independence, and safety.
- Share knowledge and information with other health care professionals and agencies to work collaboratively to deliver effective and efficient patient care.
- Builds therapeutic relationships by ensuring patients’ values and preferences are respected and balancing patient needs and choices with available resources.
- Documents assessment findings, goals, and intervention plan in accordance with Ontario Health atHome policies and the College of Occupational Therapists of Ontario standards for record keeping.
