Overview

CarePartners has embarked on a new program in partnership with the Ontario Health at Home () Central East ( Peterborough ) and we are looking for Community Support Workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy Personal Support Social Work and Community Support Services etc.) are working cohesively to meet the needs of the patient. These PSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model HISH CSWs will work 12 hour shifts and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.

The hours of work will be 7 12-hour shifts within a 2-week rotation inclusive of weekends :

Two-week Rotation

Mon / Tues / Sat / Sun

0800 - 2000 hrs

Wed / Thurs / Fri

0800 - 2000 hrs
