The Social Worker / Case Manager supports individuals and families in accessing resources and services to address their social emotional and economic challenges. This role involves assessing client needs creating care plans and advocating for client wellbeing.

Key Responsibilities :

Conduct assessments to identify client needs challenges and goals.
Develop individualized service or care plans.
Provide counseling emotional support and crisis intervention.

Proficiency in case management software and tools.

Empathy cultural competence and ability to handle sensitive situations.
