Licensed Practical Nurse $1500 Sign On Bonus

Sign‑On Bonus & Compensation

  • Base salary + $1500 sign‑on bonus

Working Hours & Rotation

  • Hours : 1900–0715
  • Rotation : Week 1 – Mon, Tue, Fri, Sat, Sun; Week 2 – Wed, Thu; Week 3 – Mon, Tue, Fri, Sat, Sun; Week 4 – Wed, Thu
  • Position Summary

    Under the direction of the Wellness Manager, the Lead Licensed Practical Nurse (LLPN) will deliver holistic, resident‑centered care within the scope of an LPN, collaborating with a multidisciplinary team.

    Position Responsibilities

  • Oversee resident admissions, discharges, and transfers
  • Monitor facility outbreaks and document to Clinical Managers
  • Support quality‑improvement projects and education programs
  • Orient, monitor, and mentor HCA and LPN partners
  • Evaluate competency assessments for HCA and LPN (medication administration, bathing, showering)
  • Assist primary practitioners on rounds
  • Apply facility policies and best practices in clinical care
  • Coordinate interdisciplinary consultations (OT, CCPS, GEM, PT, MMSE)
  • Use clinical care pathways for conditions such as SOB, UTIs, fevers, behavioral changes
  • Perform required audits and manage dietary requests
  • Report incidents to Clinical Managers, Site Directors, General Manager, AHS after‑hours
  • Follow up with Clinical Manager when AHS Case Manager is off‑site
  • Participate in IPC, OH&S, and Quality Improvement Committees
  • Respond to emergencies and maintain emergency resources
  • Adhere to professional practice standards and ensure resident dignity, privacy, and independence per the H.O.P.E. model
  • Maintain building safety after hours (procedure for unknown people, locking doors, contacting police)
  • Position Qualifications

  • LPN Certificate registered with CLPNA in good standing
  • Valid CPR certificate
  • Recent experience with elderly or disabled persons, preferably in supportive living
  • Palliative care experience (asset)
  • Leadership experience with strong collaboration skills
  • Compassionate, respectful, and diplomatic conflict‑management skills
  • Strong critical‑thinking and prioritization abilities
  • Excellent oral and written communication; additional language skills an asset
  • Flexible and adaptable to changing work environments
  • Proficient computer skills
