Job description

Implementation of a novel multidisciplinary integrated Primary Health Care Team for people after brain injury

Context

Currently there is a dearth of publicly funded Primary Care Practitioners who have the expertise to treat and manage people after brain injury. The Neurotrauma Care Pathways Initiative has co-created with key partners a Brain Injury Care Pathway that is Evidence-Based and designed to fill the current gaps in care. Once implemented, it will provide equitable care regardless of the mechanism of injury and geographic location to all Ontarians. Brain Changes Initiative has identified through clinical practice and scoping reviews of the peer reviewed literature 6 lifestyle pillars that have potential to optimize recovery after brain injury. These lifestyle pillars will fit into a model of integrated multiciliary primary care that is consistent with that suggested by the Neurotrauma Care Pathways Brain Injury Care Pathway.

Novel Care Model Implementation

Brain Changes Initiative and the Neurotrauma Care Pathways Team will collaborate on developing a publicly funded novel multidisciplinary integrated Primary Health Team model of care for people after brain injury. It will encompass the following :

  • Integrated multidisciplinary primary health care (consistent with the Brain Injury Care Pathway)
  • The best practices identified in the Canadian TBI Guideline for the Rehabilitation of the persons after TBI (consistent with the Brain Injury Care Pathway)
  • Incorporation of the 6 lifestyle pillars (Aerobic Exercise, Cognitive Exercise, Nutrition, Limiting Harmful Exposures, Sleep Hygiene and Mindfulness) using research evidence into a holistic model of care (using previously completed 6 scoping reviews)
  • A manualized standardized primary care approach that all patients would receive
  • A data collection strategy that uses the quality indicators developed as part of the Neurotrauma Care Pathways Initiative to evaluate structure, process, and outcome for the novel care model

The Scarborough East GTA Family Health Team (FHT) will incorporate this holistic multidisciplinary FHT Model. The FHT already operates in a multidisciplinary environment including family doctors and allied health care providers - physiotherapists, occupational therapists, dieticians and social workers. The Primary Care Physician overseeing all clinical activities will be Dr. Matthew Galati - primary care physician member of the Scarborough East GTA FHT and founder of Brain Changes Initiative. The implementation will be overseen by Dr. Mark Bayley and the Neurotrauma Care Pathways Team.

In order to conduct this implementation, an Implementation Coordinator with research and implementation experience will be required. The Implementation Coordinator will be engaged in the following activities :

  • In consultation with the Clinical Leads and Brain Injury Care Pathways Team, using the research evidence (recently completed scoping reviews on the 6 lifestyle pillars and the Canadian TBI Guideline) will develop a manualized multi-disciplinary Primary Care Health Team model encompassing the 6 lifestyle pillars proposed by Brain Changes Initiative
  • In consultation with the Clinical Leads and Care Pathways Team, will create the clinical implementation protocol that all patients enrolled in this novel model will receive
  • In consultation with the Clinical Leads and Care Pathways Team, will identify and develop mechanisms to collect structure, process and outcome data quality indicators
  • Will prepare necessary Research Ethics Board documents to obtain approval for this Quality Improvement project
  • Will enroll patients into the novel care model, ensuring informed consent and all implementation practices are consistent with the approved quality improvement implementation practices
  • Will ensure consistent implementation of the clinical, data collection and research protocols and oversee the implementation activities of the clinical team
  • Will conduct data analysis to evaluate the clinical and implementation outcomes
  • Will prepare an implementation report and a peer-reviewed manuscript
  • Will ensure transparency in all activities and will document all activities to allow for future scale and spread of any / all aspects of the model that show clinical efficacy
  • Will supervise and delineate duties of any / all trainees involved in the project
  • Will write any grant application(s) needed to support this project
  • This novel model of care will be implemented at the Scarborough East GTA FHT at 520 Ellesmere Rd, Scarborough ON and attendance at this site will be required 2 days per week.

    Qualifications / Skills

  • Expertise in clinical research
  • PhD in neuroscience / neurorehabilitation preferred
  • Self-motivated, detail and results oriented and able  to work independently and as a team
  • Experience supporting and managing teams
  • Desire to create treatment models and influence standard of care
  • Comfortable training and supporting health care providers to deliver the novel model of care
  • Experience supervising trainees, overseeing reviewing and providing feedback
  • Excellent verbal and written communication skills
  • Previous publications in peer reviewed journals is an asset
  • Graduate training in neuroscience / neurorehabilitation is an asset
  • Experience with brain injury / chronic condition management will be an asset
