This remote role offers a competitive salary, health, and dental benefits as well as a chance to collaborate with a growing company to expand your career experience!The Bilingual Nurse Case Manager (NCM) is responsible for managing all aspects of the patient journey in a Patient Support Program or Risk Management Program for Clozapine. The NCM will be responsible to identify, assess, and mitigate the risks associated with the development, approval, use, and distribution of pharmaceutical drugs. This program focuses on managing the potential risks related to drug safety, efficacy, side effects, and patient outcomes. Accountabilities :
- Reviews the necessary patient information and ensures all required details are accessible to the program.
- Collaborates with healthcare teams : May include but not limited to Physicians, Pharmacists, Life Labs, and other health care providers.
- Documents all interactions in electronic health record system.
- Interacts with partners to obtain relevant results and information to inform on patient health outcomes.
- Adverse Event Reporting, Risk Assessment, and Intervention identification.
- Identifies obstacles and channels this information to the Program Manager.
- Assists in training and development of new NCMs as needed.
- Supports and assists peers through regular and ongoing communication.
- Develops and maintains a positive relationship with the RMP team.
