To help fulfill its mission, LOFT is hiring a Full-time Permanent Community Clinician Job Summary The Community Clinician will work both independently and as part of an interdisciplinary team, providing consultation, direct care, capacity enhancement, and support for students enrolled at the University of Toronto who are living with mental illnesses and / or addictions. The role involves coordination, networking, advocacy, referral and linking with relevant services, supporting students in various aspects of their daily lives, and addressing mental health and addiction needs. The clinician must adhere to professional standards as defined by their respective college and LOFT Community Services. Campus Mental Health in partnership with University of Toronto The Health & Wellness Department at the University of Toronto collaborates with LOFT Community Services in providing coordinated services to support optimal mental health and substance use interventions for students identified through the University of Toronto. As a Community Clinician, you will be part of a team that coordinates a support network and linkages for youth with complex mental health, physical health and addiction challenges. A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity and self-respect. What You Will Do Intake & Referral : Conduct assessments, including OCAN’s, crisis intervention, advocacy and problem solving skills. Support & Recovery Planning : Develop individualized support and recovery plans, identify priorities, goals, and collaborate with other professionals based on initial intake and assessments. Short-term Therapeutic Counselling : relieve stress, improve coping ability, raise awareness, perspective and understanding Daily Living Support : Assist students with activities of daily living, including hygiene, medication management, cooking, cleaning, budgeting, and maintaining overall life skills. Care Coordination : Establish and coordinate support networks of social, medical, psychiatric, and community services, ensuring the development of a coordinated care plan. Crisis Intervention : Provide crisis management and assist students in creating personalized crisis plans. Advocacy & Liaison : Represent the clients and program with external agencies, families, and supports, while promoting the services of the program. Accompany students to appointments when necessary. LOFT Community Services Job Description Team Collaboration : Participate actively as a member of the interdisciplinary team, providing consultation to colleagues, identifying service gaps, and offering solutions. Documentation : Maintain detailed records, assessments, and collect statistical data in accordance with organizational procedures. Maintain strict confidentiality : follow all other required policies and procedures Program Delivery : Engage in program delivery by contributing to meetings, evaluation initiatives, and staff orientation. What You Bring : Must Haves Education : Master’s degree in Social Work (MSW), Nursing, Occupational Therapy, or a related field, with current registration with the relevant college. Experience : Minimum of three years of experience working with transitional-aged youth (16-29 years), particularly in mental health and addiction settings. Valid Standard First Aid and CPR / AED certification. Experience working with individuals experiencing homelessness, using the Housing First approach. Knowledge of community resources for mental health, addiction, and housing support. Strong understanding of recovery, psychosocial rehabilitation, harm reduction, and mental health principles. Excellent counseling, advocacy, crisis intervention, and problem-solving skills. Ability to work independently, manage time effectively, and meet program expectations. Computer literacy for documentation and records maintenance. Ability to lift or transfer as needed for the role Communication : Strong interpersonal and counselling skills for working with clients from diverse backgrounds and in sensitive areas. Collaboration : Ability to work effectively within a multi-disciplinary team, engage in program delivery, and maintain positive relationships with external partners.
