Summary
The Case Manager - Independent Living with Supports will lead the development and implementation of care plans for youth in the Independent Living Program. Responsibilities include overseeing daily operations, managing finances for the caseload, and supporting youth in achieving their goals related to life skills, employment, and education.
PRIMARY RESPONSIBILITIES of the Case Manager - ILS include, but are not limited to :
- Providing case management for youth in the program
- Managing monthly finances and expenditures for the caseload
- Collaborating with the Occupational Therapist to develop Risk Assessments, Safety Plans, and Care Plans
- Meeting regularly with the Program Manager and Occupational Therapist for planning and oversight
- Ensuring a systems approach to planning and youth-directed discharge and transition planning
- Supervising and mentoring the Tenancy Support Mentor (TSM)
- Scheduling support staff based on youths' needs
- Maintaining detailed and timely records; ensuring TSM does the same
- Providing crisis management
- Coordinating medical, therapy, and external appointments
- Communicating with the placing agency, external partners, and other Link services as needed
- Ensuring the youth's home / apartment meets residential tenancy standards and is adequately furnished
- Building relationships with landlords and following up on issues such as complaints or evictions
- Assisting youth in searching for, viewing, and applying for apartments
- Preparing leasing application packages and coordinating with management and finance departments
- Completing move-in / move-out condition reports
- Participating in staff meetings and contributing to agency functions
- Promoting positive teamwork and role-modeling
- Providing holistic services focusing on the strengths and experiences of youth
- Performing other duties as assigned

SALARY RANGE : Starting at $24.00 / hour, commensurate with qualifications. Benefits include paid sick leave, vacation, and pension.
POSITIONS AVAILABLE : One (1)
HOURS OF WORK : 37.5 hours / week, including a mix of days, evenings, and weekends
