About the Opportunity
Reporting to the Access & Flow Manager, the Community Transition Coordinator promotes a home first philosophy. The Community Transition Coordinator is responsible to lead the interprofessional team and the processes of patients who have the potential to require complex discharge needs by working collaboratively with members of the healthcare team (physicians, continuing care, nurses, community agencies and allied health professionals) and connecting patients and families with the appropriate resources to support discharge plans resulting in excellent care and improved patient experiences.
About You
We would love to hear from you if you have the following :
- Bachelor’s degree in Nursing, Social Work, Occupational Therapy or Physiotherapy
- Minimum three (3) years of related experience within healthcare required
- Current registration with the appropriate licensing body required
- Leadership experience and training considered an asset
- Experience in program planning, development, implementation, monitoring and evaluation
- Strong problem-solving, negotiation, conflict resolution and rapport-building skills
- Demonstrated strong interpersonal skills including ability to work sensitively and effectively with clients, families, various professional and community agencies and partners in a collaborative team environment
- Competencies in other languages an asset, French preferred

Hours
Compensation and Benefits
$42.19 - $53.97 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
