Position Summary
The Triage – Care Navigator is the first point of contact for individuals presenting to the HARTbeat Hub. The role completes initial, standardized assessments; coordinates immediate supports; and navigates clients to the appropriate internal and external services across health, housing, justice, and social systems. The Navigator provides brief interventions, safety planning, and warm transfers, and maintains follow‑up to ensure continuity of care to help reduce barriers using harm‑reduction, trauma‑informed, culturally safe, client‑centred principles. This role also participates in stepped‑care planning, crisis de‑escalation, and case conferences / circles to ensure integrated service delivery and health equity.

Key Responsibilities
Assessment
- Conduct initial assessment of individuals seeking access to HARTbeat Hub services.
- Refer assessed clients to the most appropriate service option based on their needs.
- Arrange referral and admissions for Medical, Treatment, Housing and other identified programs.
- Present potential clients at daily debriefing meetings and / or program application reviews.
- Forward assessments to appropriate service.
- Apply crisis de‑escalation techniques and risk assessment protocols as needed.
Client Navigation & Support
Provide proactive follow‑up and maintain regular contact with clients awaiting program admission.Meet with clients at WMS and / or HSN when beneficial to encourage engagement and readiness for programming or treatment.Conduct one‑on‑one meetings with clients and service providers prior to admission.Ensure the safety and care of clients while on duty.Provide navigation and connection to programming / services using a strengths‑based, client‑centred approach.Coordinate stepped‑care plans and warm transfers to appropriate services.Communication & Collaboration
Maintain strong working relationships with colleagues across health care, social services, and the criminal justice system to ensure coordinated and multidisciplinary client support.Foster collaborative relationships with Indigenous and non‑Indigenous agencies to support holistic wellness and coordinated service delivery.Demonstrate respect, accountability, and cultural humility in all interactions, honouring the values and traditions of the Anishinabek people.Attend staff meetings, circles, huddles, and case conferences as required.Promote anti‑oppression and health equity principles in all client interactions.Documentation & Data Management
Maintain accurate, up‑to‑date records, files, letters, and written documentation in accordance with program standards.Input data information, staff activity logs, and maintain client tracking data systems.Add all relevant client information to their file in a timely manner.Professional Development & Compliance
Maintain and expand knowledge related to pharmacology, Substance Use Disorders, Motivational Interviewing, and relevant best practices.Maintain certification and any additional assessment tool training required by the province.Current CPIC (Criminal Record Check) required.Current certifications in First Aid, CPR, WHMIS, AODA, Health and Safety Awareness Training, NVCI, MHFA and Safe Food Handler.Adhere to SKHC policies, procedures, mission, and PHIPA.Perform other duties as assigned by the HARTbeat Hub Program Manager and / or CEO.Skills And Qualifications
Social Services Diploma, Bachelor’s Degree in Human Services, or relevant equivalent experience.Strong understanding of pharmacology (including street drugs), Substance Use Disorders, and basic counselling skills.Proficient in Motivational Interviewing.Strong computer skills with proficiency in Microsoft Office applications.Excellent verbal and written communication skills; bilingualism (English / French / Indigenous language)s is an asset.Above‑average communication and interpersonal skills.Familiarity with multi‑disciplinary service environments (health, justice, community services).Job Specifications
