Triage- Care Navigator

Type : Full-Time Contract until March 31, 2028

Position Summary

The Triage – Care Navigator is the first point of contact for individuals presenting to the HARTbeat Hub. The role completes initial, standardized assessments; coordinates immediate supports; and navigates clients to the appropriate internal and external services across health, housing, justice, and social systems. The Navigator provides brief interventions, safety planning, and warm transfers, and maintains follow‑up to ensure continuity of care to help reduced barriers using harm‑reduction, trauma‑informed, culturally safe, client‑centred principles. This role also participates in stepped‑care planning, crisis de‑escalation, and case conferences / circles to ensure integrated service delivery and health equity.

Key Responsibilities

Assessment

• Conduct initial assessment of individuals seeking access to HARTbeat Hub services.
• Refer assessed clients to the most appropriate service option based on their needs.
• Arrange referral and admissions for Medical, Treatment, Housing and other identified programs.
• Present potential clients at daily debriefing meetings and / or program application reviews.
• Forward assessments to appropriate service.
• Apply crisis de‑escalation techniques and risk assessment protocols as needed.
