Job Summary: As a Case Manager you will work to coordinate a support network for clients living with mental health and substance use support needs, physical health barriers, homelessness, precarious housing, and risks of housing loss.

The position attempts to empower the client, thereby increasing their independence, dignity and self-respect.

Direct support will be provided to clients in service navigation and advocacy, linkages to physical and mental health care, goal setting, crisis intervention, and housing and shelter access.

What You Will Do: Support a caseload of clients residing and accessing services across Toronto where travel by public transit will be required; Develop a plan of support and recovery which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCANs; Provide case management to clients identifying with barriers related to substance use, mental health, physical health, housing and shelter access, justice involvement, immigration, and other intersecting barriers Promote the personal empowerment of each individual through coaching, mentoring, support and follow-up in the development of life skills, including maintaining housing, setting and attending appointments, navigating services, and managing activities of daily living; Establish and co-ordinate a support network of social, family, medical, psychiatric and other community links.

Develop a coordinated care plan as appropriate; Support in clients' long-term housing stabilization, focused on identifying, recommending and referring to housing supports, including hoarding support, personal care, services for pest control treatment preparation, addressing tenancy rights and responsibilities, and other supports as appropriate; Support clients to understand housing options and the responsibilities of tenants within a variety of residential environments, including independent rent-geared-to-income (RGI) housing, subsidized supportive housing, private market housing, congregate living, and assisted living; Provide crisis intervention and assist in the development of safe plans with clients; Mediate issues or conflicts between clients and other individuals that negatively impact clients, striving to teach conflict resolution skills to promote community inclusion and safety; Participate in planning, organizing, promoting and conducting events for the clients in an effort to address their social needs and prevent isolation; Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures; Participate as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities; orienting new staff, students or volunteers; providing other related assistance as required.

What You Bring: An excellent understanding of harm reduction, and psychosocial rehabilitation principles as acquired through a relevant degree/diploma in human services A strong understanding of housing systems within Toronto that meet the needs of clients, including rent-geared-to-income (RGI) housing access, supportive housing, assisted living, congregate living, boarding homes, and other options An understanding of housing stabilization, eviction prevention, health care connection, and how to access clinical and community-based resources for people experiencing mental health symptoms Demonstrated professional experience using harm reduction principles and strategies to support clients who use substances to develop and achieve goals, develop safe plans, and maintain housing as part of their case management plan.

Computer literacy sufficient to produce and maintain files, records or statistical information Good intake assessment, crisis intervention, advocacy and problem-solving skills Good analytical, time management and organizational skills to maintain consistency in client support Understanding of anti-oppressive principles and trauma-informed care An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities Strong skills in working within formal partnerships, including maintaining timely and thorough communication, collaborative problem-solving, and maintaining strong inter-agency relationships Excellent interpersonal, relationship-building, communication and counselling skills At least three years of experience supporting individuals in 2SLGBTQ+ communities with demonstrated skill in working with trans, non-binary and gender fluid individuals, including knowledge of relevant community services At least three years of experience working with individuals living with HIV/AIDS who have experienced homelessness and mental health and/or substance use barriers What We Offer: E-learning program available all year round Starting vacation time above minimum standard, Additional paid personal and paid sick days Professional development budget available to help you nurture and shape your career Corporate Gym membership rate with GoodLife Fitness Access to Perkopolis, a comprehensive corporate discount program Access to unlimited DEI trainings through partnership Canadian Centre for Diversity and Inclusion Work Environment: Frequent travel to Program Sites.

Ability to clean and prepare units for occupancy which may include moving furniture.

Since the workplace involves working in resident/tenant member’s own homes, staff may be subjected to second hand smoke while performing their job duties.
