Description

The Social Worker’s role is to provide information, assessment and entry facilitation into Reena’s Community Participation Supports and Residential Systems and to maintain and act as the contact for individuals who have chosen Reena to be their lead agency. The Social Worker plays an integral role in the decision-making process in relation to ongoing planning for individuals already within Reena’s system of operations, and intervenes in cases of complex need. The Social Worker provides assessment, case management, clinical and supportive counseling, service coordination, crisis intervention, advocacy, and outreach. The incumbent is further responsible for developing, leading and facilitating psychosocial and therapeutic groups.

The position maintains presence and contribution at a number of high-level internal and external committees to liaise and coordinate services within the Developmental Services Ontario (DSO) scope of authority and within the Ministry of Health and Long-term Care and the Central Local Health Integration Network (LHIN).

Note: This is a temporary position covering a maternity leave.

Key Responsibilities

1. Primary Responsibilities
  • Conduct intake assessments and facilitate the transition of individuals and families into Reena’s system of services.
  • Provides initial service options and recommends appropriate program fit.
  • Coordinate and lead 3-month follow-up meetings., Provide psychosocial and psychological assessments to support individualized planning.
  • Develop care plans and coordinated service strategies tailored to each individual’s needs.
  • Deliver clinical and supportive counselling using a range of evidence-based modalities (e.g., CBT, DBT, Motivational Interviewing, Solution-Focused Therapy).
  • Provide ongoing case management, system navigation, and short-term/long-term planning.
  • Collaborate with families, support staff, managers, supervisors, and clinicians to address complex care needs.
  • Link individuals to internal and external services and support appropriate referrals.
  • Act as a clinical resource for staff, managers, supervisors, and leadership, offering consultation and case direction.
  • Represent Reena in external forums, including municipal, regional, and provincial sector-wide committees.
  • Actively contribute to the planning and development of residential, community, and inclusion programs.
  • Handle DSO assessments on DSCIS and warm transfer requests from Ontario Health Teams.
2. Administrative Responsibilities
  • Liaise with DSO, Access Point, and other external bodies for intake coordination, vacancy management, and system-level access to care.
  • Declare Resources and manage service vacancies in alignment with Reena's internal Residential/Day planning committee and DSO protocols.
  • Participate in and lead internal and external committees as a representative of Reena.
  • Maintain awareness of legislative changes, policy updates, and system-level trends (e.g., MCCSS, OHT, MOH-LTC).
  • Monitor caseloads and document progress, outcomes, and follow-ups in accordance with organizational and Ministry guidelines.
  • Develop and manage service contracts and ensure compliance with contractual obligations.
  • Assist in preparing documentation for funding requests and business cases.
  • Provide input to Learning and Development on training content, and deliver courses based on clinical expertise and sector developments.
  • Maintain current and accurate clinical records and documentation.
  • Attend team, clinical, and planning meetings; contribute to strategic planning discussions.
  • Provide mentorship and guidance to BSW and MSW student placements.
  • Other duties as assigned.


Skills, Knowledge and Expertise

1) Qualifications

· Master’s of Social Work required
· 2+ years in developmental services or the health sector; knowledge of mental health and intake processes.
· Experience in dealing with families and counselling modalities.
· Experience and verifiable knowledge of the community served.
· Experience in performing needs assessment to identify service needs and gaps that can be addressed through psychosocial and/or therapeutic groups.
· Registered and in good standing with the Ontario College of Social Workers and Social Service Workers OCSWSSW, annual renewal required.
· Experience in family counselling, community support, needs assessments, and group facilitation.

Reena offers a variety of services including: residential support, respite programs, counselling, therapy and advocacy.
