Spectrum Health Care is currently seeking a Registered Nurse (RN) to join our team as Program Manager, Integrated Care to supportour teams in Mississauga. This is a 6 month contract with possibility of extension or permanency. This individual will be responsible for leading a diverse, multidisciplinary team that will deliver high-quality, integrated care in the community, driving both operational and clinical excellence.

Job Description
The Program Manager, Integrated Care cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.
In this role, you will ensure seamless program operations by leveraging data-driven insights to optimise service delivery, enhance quality, and achieve key outcomes. You will collaborate with external health system stakeholders to support the design and implementation of innovative programs, ensuring seamless care transitions across the broader health system.
