Position : Personal Support Worker - Home Support and Day Program (Part-Time)
Hours of Work : Variable based on client demand
Reports to : Home Support Supervisor
Deadline : Until position is filled
Position Summary :
Responsible to provide personal support services to enable client seniors to continue living with independence, and to be healthy and safe-at-home. Ensures satisfaction with the services by establishing and maintaining relationships with client seniors and their care providers and / or family members.
Responsibilities :
1. Provides personal support services to community Clients, as directed by the service and / or care plan and following procedures in the Home Support Services Handbook :
Provides personal care related activities of daily living : bathing, washroom assistance, changing incontinence products, medications, dressing, personal hygiene and grooming and dental careAssists with transferring and positioning of ClientObtains approval from supervisor for any changes to the work schedulePromotes an inclusive environment for Clients respecting abilities, age, ethnicities, languages, genders, sexual orientations2. Documents services and reports concerns and occurrences
Reports to the Care Manager and / or Supervisor about any unusual occurrences such as health concerns or emergencies, Client requests for different services, etc.Reports to Day Program Team Leader about changes in Day Programs Clients’ social, physical and mental statusClock in / out and electronically documents care plan tasks with the AlayaCare Mobile app following every visitPromotes health and safety of Clients and SelfAny other reasonable duties that may be assigned from time to time
Education :
Personal Support Worker CertificateValid First Aid and CPRSkills and Experience :
Experience working with seniors in a community program settingAbility to write clear and concise client notes in EnglishKnowledge of health care, dietary needs or limitations of individuals with special needs as a result of ageing e.g. Alzheimer's, stroke, Parkinson's diseasePhysician’s medical assessment required (A TB test form will be provided)Must be available to work on evening and overnight shiftsVulnerable Sector screening requiredProof of full COVID-19 Vaccination or Medical Exemption Certificate requiredWorking Conditions :
