New employees must provide documentation for COVID‑19 vaccination, 2‑step TB test, and proof of immunity to measles, mumps, rubella, and varicella prior to start date. Clearance from Employee Health, Safety and Wellness is required before starting.

Position Summary : This position reports to the Manager – of Ambulatory Care, Adult Diabetes Education Centre (ADEC). The successful candidate will provide effective patient‑focused diabetes education and clinical monitoring within the outpatient setting and support admitted patients requiring diabetes education.

Position Requirements :

• Baccalaureate preparation in nursing preferred
• Current Certification of Competence with the College of Nurses of Ontario
• Certification as a Diabetes Educator required
• Experience with insulin pump management required
• Minimum five years recent experience in diabetes management and education including knowledge of theoretical principles of adult learning and change theory
• Demonstrated skills in facilitating groups, teamwork, and effective communication
• Demonstrated ability to work autonomously and as a member of an interdisciplinary team
• Effective organizational, conceptual and leadership skills
• Excellent teamwork skills and ability to mentor, coach and act as a resource to inpatient units for care of patients with diabetes
