Location : Various locations across Alberta
The Clinical Pharmacist provides direct patient care through medication management and collaborates with healthcare teams to optimize theutic outcomes.
Key Responsibilities :
- Conduct medication therapy management (MTM) sessions with patients essing medication effectiveness and adherence.
- Collaborate with physicians and other healthcare professionals to develop and adjust patient medication plans.
- Provide education to patients and healthcare providers on medication management and disease state.
- Monitor patient outcomes and make recommendations for medication adjustments as necessary.
- Participate in clinical research and contribute to pharmacy practice improvement initiatives.
