This is a Contract position that would end in December 2025.
We are currently searching for our next Assistant Director of Care to lead our nursing team to deliver exemplary care to our residents and build relationships in a community unlike any other in the retirement industry.
POSITION SUMMARY :
Reports to the Director of Care
Supports the Director of Care in managing all aspects of the Nursing Department and Care within the facility to ensure residents' medical needs are met, and that the Nursing Department is run in accordance with applicable codes and regulations
DUTIES AND RESPONSIBILITIES :
Oversees the day-to-day clinical care and compliance of the unit
Ensures effective communication within unit through various tools
Creates Care Plans upon move in
Ensures that care plans are accurate and up to date, and reviewed weekly
Submits the required weekly clinical metrics to the Southlake Team on Mondays
Participates in weekly Monday rounds
Manages patient complaints and concerns – clinical
Processes and submits all Incident Reports (medications errors, falls, behaviours, etc.)
Responds to clinical emergencies and implements appropriate interventions
Liaise with Pharmacy provider to ensure best medication practices are adhered to
Liaise with NP and Physicians on patient health status and clinical interventions
Liaise with OT / PT and SLP, ensure changes are updated in care plans and communicated
Manages behaviour tracking and falls prevention protocols
IPAC / Outbreak Management Lead
Plan, organize and evaluate the nursing department to provide high quality resident care
Ensures adherence to applicable codes and regulations (i.e.. Public Health and the RHRA)
Conduct Care Conferences
Part of on-call rotation
Assists with orientation and training of new staff, as well as ongoing training and education for existing staff
Assists Director of Care with Resident admission process, including assessments, care plan development / maintenance and resident / family member orientation
Assists the Director of Care with the implementation and evaluation of required clinical programs
Assists with the preparation of daily duty assignment and re-allocation of staff to meet changing needs
Assists with staff scheduling
Coordinates nursing programs such as vaccine clinics or similar
Assists in liaising with other departments, as necessary, to ensure resident needs are met
Respond to emergencies as required
Completes other duties as assigned
PHYSICAL REQUIREMENTS :
Stretching / reaching on a regular frequency throughout the shift as an essential job function
Walking, with possibility of some upstairs walking, as a major job function
Standing, for lengths of time, up to four hours maximum. (1-2 hours usual) as a major job function
Must be able to move, lift or push equipment and supplies of various sizes and weights
Must have visual and hearing acuity to recognize the need of assistance required by residents or staff
Health and Safety :
Is aware of employee's responsibilities and follows all health and safety policies and procedures as set out in the employee safety program handbook and departmental manuals
Works safely to reduce the risk of injury to self, co-workers, and Residents
Does not operate or use faulty equipment
Wears personal protective equipment (or clothing) as required by task, S.D.S. or residence policy
Participates in fire safety demonstrations and fire drills and knows the residence fire and disaster plans
KNOWLEDGE SKILL AND EXPERIENCE :
Minimum designation of Registered Practical Nurse (RPN), possessing a current Ontario Certificate of Competence for the College of Nurses of Ontario
Proficient in Microsoft Word and Excel
Excellent communication skills, both written and oral
Excellent analytical skills and attention to detail
Current and annual renewal of certification in CPR and First Aid
Demonstrates initiative, good judgment and leadership abilities as well as supervisory experience
Familiar with RHRA Standards & Responsibilities
Excellent customer service skills – possess a can-do attitude to promote a positive work environment for the entire team
