Job PurposeThe Care Aide conducts home visits to provide chronic and acute care and wellness services to community members. The Care Aide applies a structured client assessment process that includes on-going reassessment to determine client needs, family supports, and service allocations. Following the individual client assessment and care plans, the Care Aide provides medical support to clients with their daily living activities, including personal, social, physical, and spiritual needs. The Care Aide ensures all clients are treated fairly with kindness, dignity, and respect.This position within the Health Services team and has an overall objective to provide the community of Saulteau First Nations (SFN) with effective home services that contribute to and enhance the health and well-being of the SFN’s elders. Duties and ResponsibilitiesAs Scheduled by the Home Care Nurse Lead : Assists clients with daily activities (eating, dressing, grooming, and personal hygiene)Performs or assists client with light meal preparation, encouraging nutrition and healthy food selection.Checks and records relevant vitals / measurements (BP, temperature, blood glucose, fluid intake and output); collects specimens as required.Assist nurses with wound care and treatments as required.Ensures proper usage of prescribed medications (does not administer medications)Monitors progress, symptoms and behaviour and reports key observations or client safety concerns or emergencies to Home Care Nurse Lead. Assists clients with mobility issues (weigh, lift, turn and position them)Provides emotional support and companionship.Advocacy of clients needs and supports.Assists with respite and palliative care support. Performs care-based cleaning including emptying / cleaning commodes.Administers First Aid in an emergency.Maintains a log of client’s home visit and duties that are performed.Maintains supplies inventory and performs sterilization of equipment.Any additional duties as requiredRequired Qualifications and Certifications Licensed Care Aid Certification or Home Support CertificateMust have current registration with the BC Care Aide RegistryExperience working with an EMR (electronic medical record) system is an asset.Experience working in a First Nations rural community is an asset.Valid First Aid and CPR-C.Valid BC Driver’s License with access to own vehicle.Vulnerable Sector Police Information Check.Required Knowledge, Skills, and Abilities Knowledge of SFN community, language and culture is an asset.Excellent time management and organizational skillsAbility to lift or move up to 50lbs is required; must be physically ableto perform the duties listed above.Ability to respect client’s personal property rights and maintain clients’ rights to privacy and confidentiality.Ability to follow a schedule and adapt to changes based on clients’ evolving needs.Ability to follow established safety practices in the performance of duties to ensure personal safety.Ability to demonstrate patience, tact, and a willingness to interact with clients, difficult clients, other family members, and the public.Ability to build trust, serve as a positive role model and maintain confidentiality.Work ConditionsTravel within SFN community and surrounding community.Work is performed primarily during the hours of 8 : 00 a.M. to 4 : 30 p.M. Monday through Thursdays.
