Evaluates, Assesses, and plans nutrition care to meet patient nutritional needs:
- Assesses patient's nutritional status using documented medical information and established nutritional standards
- Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
- Identifies inpatient at nutritional risk and documents recommendations.
- Diagnose and develop intervention for patiens with malnutrition
- Participates in physicians’ rounds, discharge planning rounds, and conferences with nutritional implications.
- Conducts education programs for patients, community groups, and staff.
- Precept and mentor new staff or existing staff in clinical specialty areas.
- Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broadenerspectrum patient care.
- Cross-Covers Broader spectrum of patient care areas and subspecialties as needed.
- Performs other duties required or assigned.
Qualifications
A Bachelor's degree in Nutrition from an accredited college/university is required.
