In your role as a Clinical Pharmacist, your day-to-day responsibilities will include :
- Conducting the formal process of medication reconciliation in accordance with CareRx policies and procedures for all CareRx clients pending admission to long-term care, retirement living, and other facilities, with a goal to improve patient outcomes.
- Providing comprehensive pharmaceutical care in collaboration with the interdisciplinary team and offering recommendations to clinicians.
- Working with the healthcare team to develop strategies to optimize resident care in transition and improve outcomes.
- Producing quality metrics related to clients and medications in transitions and supporting departmental initiatives to deliver quality patient care in accordance with CareRx and Department policies and procedures.
- Liaising between the dispensing pharmacy, manager, and staff to ensure optimal pharmacy services are provided to each facility.
- Please note that this position is salaried, and while typical hours aim to be 9 AM to 5 PM, they can be variable depending on the needs of the organization.
To be successful in this role, you must :
- Be registered in good standing with the Ontario College of Pharmacists (RPh).
- Have prior experience in long-term care pharmacy or hospital pharmacy.
- Be able to effectively use technology to support medication management, monitoring, and documentation.
- Possess excellent written and verbal communication, interpersonal, and presentation skills.
- Demonstrate proven analytical and problem-solving experience.
- Be able to organize and prioritize work effectively.
- Be self-motivated and results-oriented.
- Work effectively within the organization to establish relationships, build trust, and influence appropriately to drive performance through learning.
- Be comfortable working in a 100% paperless environment.
