Reporting to the Director of Wellness, the Medication Care Partner is a critical role within our communities that impacts the lives of residents.

Responsibilities

• Administer medication according to relevant legislative requirements, policies and procedures.
• Review residents’ Support Plans and provide assistance for daily living activities such as bathing, toileting, dressing, grooming, mobility, continence management, laundry handling.
• Monitor and document residents’ vitals, weight, behavior, mobility and safety; report changes to appropriate team members.
• Escort residents to social activity programs, appointments and dining rooms.
• Promptly respond to nursing call systems.
• Perform other duties as assigned.
