OVERVIEW
East Toronto Health Partners (ETHP) is the Ontario Health Team (OHT) serving East Toronto. We are a group of more than 100 community, primary care, home care, hospital and social services organizations in East Toronto, as well as patient / client, caregiver and community advisors, working together to better connect local care and services. We work together as an integrated team to make it easier to navigate the healthcare system and transition between healthcare providers and places. Our goal is to build a healthier and more equitable East Toronto – enabling every person and neighbourhood to thrive.
Using a learning health system approach, this position will be responsible for supporting project / program / change interventions, implementation and operations of priority initiatives such as integrated care pathways in collaboration with the build and implementation teams.
The incumbent will work collaboratively with key interested parties, hospital partners, primary care, community members and front-line staff across the East Toronto Health Partners Ontario Health Team (ETHP) to facilitate positive change. Supporting alignment and integration with other OHT initiatives, program evaluation and quality improvement processes, and supporting team-based care planning and operations for cross-organizational accountability of ETHP priorities.
This is a one year contract position with possibility of extension.
PRIMARY RESPONSIBILTIIES
GENERAL ACCOUNTABILITY

NATURE AND SCOPE
ETHP provides a comprehensive basket of health and social services, tailored to meet changing local needs. ETHP provides primary care to acute care, food security to supportive housing, home-based healthcare and community support services, birth to end-of-life care, and settlement to employment. The partnership is built on a legacy of trust in East Toronto, with a multi-decade track record of delivering ‘made in East Toronto’ solutions together with our partners.
A specific focus of this position is supporting the evolution of integrated care program planning and design into operations for priority populations and communities. This includes the ongoing development of homecare modernization, integrated care pathways for chronic diseases, and integration / alignment of these programs within ETHP. The position will also contribute to the design, planning and operations of technology enablers and workflows to support effective and connected care teams.
The Project Specialist will have extensive experience working with interdisciplinary teams to build a continuous improvement culture. The candidate will draw on their experience with structured and practical approaches to improvement that helps build systems position requires strong project management, operations, understanding of implementation science, quality improvement, and relationship building skills.
The incumbent also will participate in ETHP Virtual PMO Group and partner engagements and othergroups as required to ensure the initiatives align with other OHT initiatives.
SPECIFIC ACCOUNTABILITIES
Reporting to the Manager, OHT Development and Care Integration, the Project Specialist will be responsible to support coordination, implementation and operations of priority initiatives in collaboration with the project teams from different ETHP partners.
o Design and build components of the program including; referral pathways, intake / assessment processes, planning / triage and coordinated care planning documentation to support integrated care planning and delivery.
o Develop, plan and improve tools and processes to support centralized intake, sharing of clinical information / records, planning for seamless transitions to programs and services in the community
Additional roles and focus areas may be added through mutual discussion as ETHP matures as an accelerated OHT.
