Job description

TEMPORARY UNTIL MARCH 31, 2026

As a CMHA Health Centre multidisciplinary team member, the Community Outreach Coordinator works to improve the health of individuals, populations and communities informed by key data sets including client, agency and community-based surveys, performance expectations outlined within related funding agreements, the needs of identified priority population(s) and the social determinants of health.

RESPONSIBILITIES & DUTIES :

  • Liaise with community agencies and groups to identify possible outreach opportunities.
  • Conduct and / or work in partnership to conduct community needs assessments, and analyze assessment findings.
  • Ensures that monthly, annual and other required reporting documentation are completed for the health centre and other community / partner initiatives.
  • Identifies funding opportunities and assist in proposal development.
  • Provides minor assessment and educational counselling for clients in health‑related programs as needed.
  • Use health promotion theories and strategies to develop health promotion education and community initiatives that meet identified needs for priority populations.
  • Identify population demographics.
  • Identify language preferences / barriers and other EDI components.
  • Coordinate the planning, development, implementation, and evaluation of educational and outreach materials, workshops and programs.
  • Identify emerging issues in the delivery of primary care services to high risk, vulnerable populations / neighborhoods e.g. homeless / precariously housed, newcomers / immigrants, agri‑food workers, substance misuse, designated high priority neighborhoods.
  • Develop / Coordinate programming that reaches these high risk, vulnerable populations.
  • Identify key drivers of health inequities and develop / implement strategies to support improvement initiatives to address inequities.
  • Support the delivery of culturally competent care.
  • Develop organizational capabilities to meet patient needs and preferences.

Training

  • Develop and deliver education workshops to target populations and service agencies based on community needs assessment and emerging issues.
  • Involve community partner agencies, health care providers, service users, and stakeholders in the planning and delivery of educational / program materials and services.
  • Provide training to the trainers in other related programs.
  • EDUCATION & SKILLS :

  • University degree in biological, social, behavioural sciences or health promotion or education. Master’s degree considered an asset.
  • Three (3) year’s health promotion / community outreach experience in diverse community settings.
  • Experience working with marginalized populations, specifically clients with moderate to severe mental health concerns.
  • Knowledge of the Social Determinants of Health and the relationship to the health and wellbeing of individuals.
  • Experience in program development.
  • Knowledge and experience in developing awareness campaigns.
  • Group facilitation skills.
  • Established relationships with community partnerships.
  • Equity, Diversity and Inclusion knowledge.
  • Knowledge of community supports available to marginalized clients.
  • Experience working within a multidisciplinary team.
  • Proven ability to effectively work within short timelines.
  • Knowledge of Recovery Orientated Approach and Concurrent Disorders.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
  • Hours of Work : 35 hour work week. The agency’s regular hours of operation are Monday to Friday, 8 : 30 AM to 4 : 30 PM. Flexible hours are required to meet client and service needs.

    Salary range : Grade 9, $37.84 – $44.81 per hour (Per 2022 - 2026 Collective Agreement).
