The Director of Clinical Practice contributes to clinical program implementation (e.g., chronic disease management, wound care, deprescribing / medication optimization, antimicrobial stewardship supports), employee coaching and education, and continuous quality improvement to improve outcomes and resident / family experience. This role includes responsibilities such as coaching, performance support, and involvement in hiring and termination decisions for Clinical Care Leads, consistent with organizational policy and employment legislation.
