Temporary Case Manager Nurse (RN) Fully Remote (California)

A managed care organization is seeking two experienced Temp Case Manager Nurses (RN) to support complex member populations through coordinated, compliant, and patient-centered care. This role partners closely with members, providers, and interdisciplinary care teams to ensure high-quality, compliant, and patient-centered care.

Highlights

️ 100% Remote opportunity
️ $55 / hour
️ 5-month temporary assignment (cannot guarantee temp to hire)
️ Active California RN license
️ 3+ years RN clinical experience (hospital or clinic)
️ 3+ years in Case Management / UM / Discharge Planning
️ Experience with Medicare & Medi-Cal populations, preferred
️ Previous experience in CALAIM or ECM, preferred

What You Will Do

Assess member needs and develop individualized care plans

Coordinate care with members, caregivers, PCPs, and ICTs

Support SNP and vulnerable populations per CMS & DHCS guidelines

Monitor progress, document care, and escalate high-risk issues
