Job description
A local health authority in Canada is seeking an Outreach Worker to support individuals and families within the Long Term Care Home Health team. Responsibilities include providing administrative, financial, and clinical support, monitoring clients' health, and liaising with community resources. The ideal candidate will have a diploma in Community Social Services and experience working with diverse populations. This part-time entry-level position offers a salary of CAD $31.56 to $33.28 per hour.
