Specialized Support Services Coordinator
This vacant position requires working with clients with complex needs, providing comprehensive supports that includes crisis intervention, housing assistance, system navigation, and case management. The Specialized Support Services Coordinator works collaboratively with the Specialized Support Services team to set up and provide wrap-around services to support client needs. Developing and maintaining a strong working relationship with health and social service community partners is foundational to this position.

Primary Duties
Identify and facilitate client access to community resourcesCompletes intake assessments using LEOs Standard Intake Form and the Specialized Support Services Intake Form, financial assessment forms and when required interRAI tools
Documents and maintains client files in accordance with LEO / program expectations, including statistical entries and re-assessments at a minimum annually or as neededAssesses client needs and develop an appropriate plan for possible housing and wrap around support based on the need of the client and their goal(s)Assists with referrals to secondary and tertiary health care servicesCoordinates client access to and navigation through the healthcare systemLiaises with health care providers and community agencies as required to coordinate wrap around services for the clientAdvocates for clients to ensure that they receive the community supports, and health and social services they requireWhen necessary, supports clients with services such as errands, attendant shifts to appointmentsManages a caseload, plans for discharges, determines priorities, meets deadlinesTrack and reports active clients and case loads to the Manager monthlyTeamworkWorks collaboratively with clients, families, community partners, and other LEO team members

Working in an office environment.The position also involves frequent travel throughout Lambton County to meet with clients and / or caregivers for assessment purposes.Occasional travel is also required to attend training and / or meetings outside of Lambton County.Overtime as required and approved by ManagerDealing with complex clients or situations may be part of a working day.Manual dexterity required to use desktop computer and peripheralsRequirements
Completion of a satisfactory Criminal Reference Check and Vulnerable Sector ScreenPost secondary education in social services, nursing, gerontology, or equivalent. Social Service Worker education and experience preferred.Excellent verbal and written communication skills and the ability to communicate effectively with individuals at all levels of the organizationPrior experience and knowledge of electronic medical records, especially with AlayaCare is preferredProficiency in the use of computer hardware and software, particularly with Microsoft Word, Excel, and OutlookDisclaimer
