We are looking for RNs and RPNs who desire great flexibility in their schedules.
This is a Part-Time / Casual position in the Sarnia area.
Hours of work are within a 2-week rotation inclusive of weekends and on-call work.
The position will be within our Visiting Nurse Program offering service in the patient's home.

Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.Working with patients ( both in-person and virtually ) to provide exceptional nursing servicesProviding services one-on-one with patients in a home settingCommunicating and liaising with patients, family members and others within the interdisciplinary care teamPerforming other clinical duties as requiredWhat You Bring
Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diplomaCurrent College of Nurses of Ontario (CNO) registration in good standingA strong desire and commitment to making a difference in the lives of our patientsExceptional interpersonal skillsA valid driver’s license and vehicle as regular travel is requiredExcellent English verbal and written communication skillsCPR CertificationCurrent immunization record including 2 step TB testTo protect our patients, we require a current vulnerable sector check and a clear background checkCarePartners In Your Community
In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
