Reporting to the Manager of Dialysis, Diabetes Education, OPNC and Paediatrics, the successful applicant will be responsible for the provision of diabetes education to patients in the Diabetes Education program. This will include assessment of patient’s educational needs, implementation of the education in the most appropriate format and evaluation of the effectiveness of the education. The successful applicant will work as a member of the Diabetes education team.
Mandatory Qualifications
At least 2 years recent experience working in Diabetes Education Clinic settingGraduate of a University Nursing Program (BScN)Current Basic Cardiac Life Support (BCLS) certificationRegistration (in good standing) with the College of Nurses of OntarioEducation Requirements
Certification in Diabetes Education preferred or actively pursuing Certified Diabetes Educator (CDE) designationCertification in Insulin Pumps or actively pursuingMandatory Competencies
Demonstrated ability to provide diabetes education, including management techniques to a variety of adult patients with Type 1 and Type 2 diabetes and those with gestational diabetes using a variety of learning styles incorporating cultural knowledge and low‑literacy learningCurrent in evidence based best practices and clinical practice guidelinesDemonstrated ability to provide diabetes education, including management techniques to children with Type 1 and Type 2 diabetesAbility to engage in health promotion activities on and off siteHighly developed communication, presentation and group facilitation skillsExperienced in the use of insulin pumps and the continuous glucose monitoring systemExceptional interpersonal and communication skills with a team focusSelf directed individual with the ability to work both independently and in collaboration with other staff members as a part of program planning teamDemonstrated commitment to continuing education and personal development in nursingCreative problem solving abilities with a focus on solutions and patient friendly approachesDemonstrates commitment to exceptional customer service with patients, families, staff, colleagues and community partners with an awareness of cultural, racial, economic and social diversityProven critical thinking skillsProven organizational and time management skills with ability to multi‑task and set priorities in a rapidly changing environmentWell developed negotiation and conflict resolution skillsAbility to establish positive and proactive work relationships with external partnersDemonstrated ability to be a coach, teacher and mentor to colleagues and studentsDemonstrated excellence in patient assessment and care planningReliable individual with excellent attendance record and ability to work flexible hours during the day and early eveningPrimary Responsibilities
