Medicine Program, Trillium Health Partners

Trillium Health Partners (THP) is the largest community-based academic health network in Ontario serving over one million residents in the communities of Mississauga, Peel Region and West Toronto. The hospital encompasses three main sites – Credit Valley Hospital (CVH), Mississauga Hospital (MH), and Queensway Health Centre (QHC) – offering a full range of acute care hospital services, as well as a variety of community-based specialized programs including the Peel Regional Cancer Centre.

The organization is an affiliated academic teaching centre with the University of Toronto. The Regional CKD Program at THP is the larger of the two programs within the Mississauga-Halton Local Health Integration Network (LHIN). It provides acute nephrology consultation and management at both Credit Valley Hospital and Mississauga Hospital sites. This program also delivers the full spectrum of outpatient renal care including CKD consultation and follow-up, home dialysis modalities (peritoneal and home hemodialysis) as well as in-centre hemodialysis services. In addition, the program offers follow-up care for renal transplant patients close to home.

The Division of Nephrology, Medicine Program is currently recruiting for one (1) 0.5 FTE Associate position to start in Summer 2025 to join our team of twelve nephrologists with well-established practices. Hospital-based inpatient and outpatient renal care, including in-centre HD, is shared equitably amongst the nephrologists. In addition, participation in the General Medicine call schedule is required. All Division members are also expected to establish an office practice in the community to help meet the growing needs of the population. Support is available within the division to join an already established community practice as well.

The successful candidate will also promote and exemplify excellence in professional practice and conduct and be licensed or eligible for a license to practice in Ontario with accreditation in Nephrology by the Royal College of Physicians and Surgeons of Canada or its equivalent. For the successful Associate candidate, they will have evidence of an academic interest and expertise and be eligible for an academic appointment at the University of Toronto, Faculty of Medicine. Finding a candidate with additional skills and experience, such as an extra fellowship in glomerulonephritis or home dialysis or other academic training, research interests such as leadership interest and experience, or a career focus in medical education, is a particular priority. Excellent interpersonal skills and a commitment to team-based and inter-professional care are essential.
