Job description

The Prince George Native Friendship Centre (PGNFC) delivers programs and services through a lens of reconciliation, as we have been doing since the inception of the PGNFC for the past 50+ years. A key component of this journey is ensuring the voices of each person is heard and included in our trauma informed and culturally dynamic approach; this perspective is critical to reconciliation. Come join our House of Friendship Outreach team as an :

Community Support Worker – FT and PT

House of Friendship Outreach Program ($28 / hr. - $30 / hr.)

The Community Support Worker plays a key role in supporting individuals to improve their health, wellbeing, and social inclusion. The role focuses on building strong relationships within the community, identifying needs, and connecting people to appropriate local services, groups, and resources. The postholder acts as a bridge between individuals, communities, and service providers to reduce isolation and promote independence.

SCOPE OF POSITION

  • Work with individuals in the community selected by Program Coordinator and Management.
  • Builds Connections :   Helps individuals find social groups, sports teams, classes, and volunteer opportunities that match their interests, using "social prescribing" to address social determinants of health.
  • Develops Wellness Plans :   Works with the person to create personalized plans for social, emotional, and physical well-being, identifying goals and strengths.
  • Provides System Navigation :   Connects individuals to essential community services, including housing, employment, transportation, mental health support, and food programs.
  • Offers Ongoing Support :   Provides regular check-ins and adjusts plans as needs and goals change, ensuring the supports are effective.
  • Make daily reports  of activities and time spent with client.
  • Create and maintain good public relations.

Qualifications :

  • A diploma in community social work services preferably, social work or a combination of education and experience.
  • Willingness to work with at risk adults in a variety of settings, as well as clients with diverse abilities.
  • Class 4 drivers license but will consider applicants with class 5 if willing to obtain class 4 within 3 months of hire.
